Prior to seeing each new patient, we need to gather a little patient information. Here are the forms we need for your upcoming appointment. Please call our office to schedule a new patient exam before you fill out the new patient forms. Once you have called us, please click on the appropriate form and fill it out prior to your visit. Alternatively, you may fill this out at our practice; however, please arrive at least 15 minutes prior to your appointment in order to have time to provide the necessary information.

We value our patients’ privacy and would never sell or share your personal patient information. It is used solely for the purpose of providing you with excellent service, speeding up your visit and assisting with your insurance. See you soon!

PATIENT INFO – YOUR FIRST VISIT

We look forward to meeting you at your first visit to our office. During this time, we will review your patient information and help you get acquainted with our office while learning about your orthodontic treatment. We understand that each patient’s treatment is different, and we promise to give you the personal attention you deserve. Your first visit will consist of an initial examination and a discussion of your patient information and potential treatment options. This important visit will allow us to determine if this is the right time to begin orthodontic treatment.

OFFICE POLICIES 

The orthodontic treatment process involves a series of orthodontic visits. Our team values the relationship that we will build with you and your family throughout that process and want you to be confident that you are receiving the best orthodontic care possible. We value your time, and we know you want to make the most of your visit. In order to make the most of your visit, please review our patient information and office policies below.

  • Office Hours: These times are subject to change, please call ahead. If you have not done so, please provide your email address to the front desk so that we can remind you of appointment times and important updates.
    • Georgetown: Monday – Thursday 8 a.m-4:30 p.m.
    • Moncks Corner: Monday and Thursday 8 a.m-4:30 p.m.
    • Summerville: Tuesday and Wednesday 8 a.m-4:30 p.m.
  • Emergency numbers are listed on the office answering machine and are for true orthodontic emergencies.
  • You may experience some soreness in your teeth as they begin to move. It can last two to three days. If you wish to take something for the soreness, pain relievers such as Advil, Aleve or Tylenol are very effective.
  • It is important for patients to be seen on a regular schedule to keep treatment moving, generally every four to eight weeks unless otherwise stated.
  • Missed appointments can cause treatment to be prolonged and can result in additional costs.
  • If you are wearing an appliance or night guard, always bring it with you to every appointment.
  • Please arrive on time for your appointment. If you are later than ten minutes, you may have to wait. If you are later than fifteen minutes, you will be ask to reschedule.
  • Please call 24 hours in advance to cancel any appointments. We reserve the right to charge your account an additional $25.00 fee for no call/no show appointments.
  • Pre-pasted toothbrushes are provided in our office for patients to brush before being seen.
  • There may be an additional charge for more than four broken braces and two broken wires during treatment time. Please try to follow the food list to minimize breakage as this also slows down the treatment.
  • If the patient has a broken brace or wire, please call before appointment date so we may determine if more time is needed for repairs.
  • The patient is responsible for the full amount charged; however, we do accept insurance and will file a claim for you as a courtesy.
  • Account must have a zero balance at time of retention.
  • Accounts are divided into monthly payments and are due monthly even if your appointments are more than four weeks apart.
